Signed forms and waivers

Have customers sign a waiver, photo release or agreement on your site with a finger or mouse. You and they each get a PDF copy.

A signed form shows the wording you want people to agree to, like a waiver or a photo release, and a box to sign in. They sign with a finger on a phone or with a mouse, or type their name if they'd rather. You get the signed copy as a PDF, and so do they.

Start from a template

  1. Click Pages in the left menu, then New page.
  2. Pick Activity waiver, Photo release or Waiver for a child, and give the page a name.
  3. Open the page's Form section and read What they agree to. Change the wording to fit your business.
  4. Publish.

Have your lawyer check the wording before you use it. The templates are starting points, not legal advice, and what a waiver can do differs from state to state. A waiver for a child is signed by a parent or guardian, and some states limit what it covers.

These pages are kept out of search results, since people reach them from a link you send or from their booking.

Turn any form into a signed form

In any Form section, turn on Ask for a signature and write the wording in What they agree to.

  • Leave a blank line between paragraphs. A line starting with ## is a small heading, and **words** are bold.
  • Turn off Ask for a message if the form is only for signing.
  • Questions you add under More questions, like an emergency contact or a child's name, go on the signed copy too.

Before they can send it, people must sign and tick "I have read and agree to the above."

Ask everyone who books to sign

An appointment or an event can ask people to sign a form, such as a waiver for a class:

  1. Click Lists, then Appointments or Events, and open the one you want.
  2. Under Form they sign, pick the page. Only pages with a form that asks for a signature are listed. For an event, sign-ups on your site have to be on.

After booking, they see "Please sign the … before you come" with a Sign it now button. Their confirmation email and reminder have the same link, and so does their booking's own page. Signing from that link keeps the signature with their booking.

In Bookings, each booking shows whether it's signed, with its PDF. An event's sign-up list says who has signed, and so does its printed list. For someone who booked by phone, open the booking and click Copy the link to sign to send it to them.

What's kept with each signature

Each signed form arrives in your Inbox, marked Signed, and in your email with the PDF attached. The signer gets their own copy by email. Open it in the Inbox to see:

  • the signature, who signed and when
  • the page, their IP address and their browser
  • the wording exactly as they saw it, in the language they signed in, with its fingerprint (a code that changes if a single letter of the wording does)
  • whether the wording was the same as your page's when they signed
  • Download PDF

Signed forms are kept for as long as you have your account, even if you delete the message or the booking. When you leave, Download your site includes every PDF.

Don't collect health information

Don't use signed forms, or any form on your site, for medical history, conditions, medications or treatment. Your site isn't built to protect health information and isn't covered by HIPAA. If you need it, ask in person or use a service made for it. A waiver can still ask people to tell your staff about anything that affects their safety.

What it costs

Signed forms come with the Business plan or the Signed forms add-on ($2 a month). See The Business plan.

If your site no longer has signed forms

If your site loses them (you go back to the base plan, or turn the add-on off), the forms already on your site still ask for a signature until you change them. For 30 days we keep signed copies as before, and each message's email reminds you. After that, people can still send the form and you still get their message, with the name they typed, but no signed copy or PDF is kept.

To fix it, turn off Ask for a signature on those forms and publish, or turn signed forms back on from your dashboard's Plan page. Until you do, a publish check stops you publishing a form that asks for a signature.
