Bring in a form you already use

Have a paper, PDF or Word form, like a waiver or a registration? AI copies its questions, and the wording people sign, into a form on your site. You check everything before it goes live.

If you already have a form on paper, as a PDF or in Word, you don't have to type it in. Choose the file and AI copies it into a Form section: its questions and, if it has a signature line, the wording people agree to. It's included in your plan, with no extra charge.

Fill in a form from your file

  1. Open the page where the form goes, or add a page (the Activity waiver template is a good start for a waiver).
  2. Open its Form section and click Fill in from a form you have.
  3. Click Choose files and pick your form:
    • Photos: one per page, up to 4. Lay the form flat and shoot straight on, in good light.
    • PDFs: up to 4.
    • A Word file (.docx): one on its own. For an older .doc file, open it in Word and save it as .docx or PDF first. A Google Doc can be downloaded as either.
  4. Click Read this file and wait about a minute.

The form's questions replace the ones in the section. Your button, thank-you message and other settings stay as they were.

What's copied, and how

  • Name and email are on every form already, so they aren't copied as questions. A phone number turns on Ask for phone number.
  • Other questions keep their order, with the right kind of answer: short or long text, a number, a date, yes or no, one choice from a list, or Several choices for "tick all that apply". Long questions are shortened to fit.
  • A signature line turns on Ask for a signature. The wording above it (the waiver, release or terms) is copied word for word into What they agree to.
  • A comments box becomes the message box.

A form holds up to 20 questions. If yours has more, the window says how many were left out.

What's left out

The window lists anything it didn't copy, and why:

  • Health questions, like medical history, conditions or medications. A website form can't collect health information. Ask in person, or use a service made for it. See Signed forms and waivers.
  • Card, bank and ID numbers. Take payments with Request a payment instead.
  • Office-use-only boxes, a witness's signature, and initials beside each paragraph (one signature covers the whole wording).

Check it before you publish

AI can misread small print or a blurry photo, so read each question and the wording against your form. If the form is a waiver or a release, have your lawyer check the wording before you use it. Nothing is on your live site until you publish.
